How to report
Include people, dates, locations, events, and how you became aware. Share evidence only when legally authorized and never access restricted records.
Confidential handling
Information is shared only as needed for assessment or investigation, subject to applicable law. You may request confidentiality.
Protection from retaliation
Anyone reporting or supporting an investigation in good faith should not face retaliation. Anonymous reports require sufficient facts.
What happens next
Reports are logged, screened for urgency and conflict, and assigned for appropriate independent review. Follow-up information may be requested.
